Kepro and Symmetry have partnered to deliver ongoing supportive services to the aging population. We facilitate customizable healthcare solutions for our clients in partnership with Kepro.

C.W. Williams

A local Federally Qualified Health Center (FQHC) providing quality primary care and supportive services for the un/under insured. Symmetry sends referrals along with providing information regarding medical as well as community related information in order to better inform the community of health related issues.

Outreach

We provide outreach services to persons who have tested positive for COVID-19 and/or families who have been negatively impacted by COVID. We ensure that they are connected with the necessary resources such as testing/contact tracing, primary care and related support services through telephonic outreach or face-to-face encounters.

 

Southminster

Due to COVID-19, there has been an increase in isolation, which has caused a dramatic increase in anxiety and depression. This is especially true for our senior population. We have partnered with Southminster to address this isolation with a focus on mental health support to seniors and their caregivers; improve social engagements with seniors through the use of unique interactive activities; and, increase their technological skills to gain greater access to services.

Case Management

We provide supportive care services, which includes but are not limited to scheduling and conducting social activities for senior residents; providing assessment and referrals; assisting residents with income verification for lease renewal.

Supportive Services

Symmetry works with partners to deliver an innovative program to provide supportive services to more than 300 clients at Prosperity Creek Seniors Apartments and Steele Creek Senior Apartments. This includes social engagement and technology development programming.
